Service Request Order FormsOn-line Ordering and RetrievalTo use our On-line Ordering and Retrieval system (dnaLIMS), click on the On-line Ordering and Retrieval link above. You will be asked to log-in or create a log-in account. We may have already assigned you a log-in name and password. If you have already received a log-in name and password from us, we recommend that you change your password once you have entered into dnaLIMS for the first time. Note: Your Log-in and Password are case sensitive. Once in dnaLIMS, you can create an on-line order by clicking the 'Sequencing Request Form' button. You will then be prompted to enter the total number of reactions. From there, you will be able to input your sequencing request information in a new and easy to use form (that offers indexing and fill-down functions) for both small and large orders. When done entering your sequencing request information, just click the 'submit' button and print 2 copies of your request form (1 for you and 1 to send with your DNA samples).Printable FormsDavis Sequencing provides our customers with simple printable forms.U.S. University Printable Form U.S. Government (Public) & International University Printable Form U.S. & International Commercial Printable Form Downloadable FormsDavis Sequencing provides our customers with downloadable forms. These forms are in PDF (Portable Document Format) and you will need to have a computer application, called Adobe Acrobat Reader (which is FREE), installed on your computer to view it. Once you have installed Adobe Acrobat Reader you can select the desired file.
